Executive, Social Work

The Executive, Social Work will assist the Head of CAREs to facilitate the delivery of social services to our clients. He/She will also collaborate with a multi-disciplinary team of Doctors, Nurses, Medical Social Workers, and other Allied Health Workers from other healthcare institutions to ensure the smooth transition of the clients into the nursing home.

Key Responsibilities

• Identify and assess clients’ needs in order to provide solutions and manage the service offerings.
• Gather social and financial background of the resident and/or the family and liaise with multi-disciplinary teams on the continuing care for residents.
• Single Point of Contact and liaison for residents, family, and healthcare institutions to coordinate referrals and process applications of potential residents, including financial subsidies.
• Handle referrals, enrolments, enquiries, and admissions to discharges of residents.
• Develop and maintain effective communication resources to promote the business and services.
• Build effective relationships with medical institutions, physicians, specialists, and relevant government bodies with the objective of increasing our utilization and capacities of our facilities.
• Market the services through relationship management, facilities familiarization trips, and close customer support.
• Assist in establishing, implementing, and reviewing policies, procedures, and protocols to ensure smooth and effective execution and performance of all marketing and advertising programs.

Key Requirements

• Degree or Polytechnic Diploma with social work exposure preferred.
• Proficient in English and Mandarin to facilitate communication with the residents who do not understand English.
