In this role, you will have the opportunity to support patients with their healthcare and financial needs, assist with social service case management, and work alongside a team of healthcare professionals to provide holistic care. You’ll be involved in various tasks such as counseling patients on financial assistance schemes, processing applications, supporting care coordination, and contributing to the development of social work services.

Key Responsibilities:

1. Conduct assessments to identify the needs and challenges of patients/clients, and develop appropriate intervention plans.
2. Provide support and guidance on healthcare options, financial assistance, and relevant services.
3. Assist with healthcare discussions and ensure compliance with regulations and guidelines.
4. Collaborate with healthcare providers, community organizations, and social service agencies to coordinate care.
5. Maintain accurate records and track the progress of patients/clients.
6. Provide administrative support for healthcare initiatives and programs.
7. Coordinate patient referrals, follow-ups, screenings, and other necessary healthcare services.
8. Assist patients with using healthcare-related applications and provide technical support when required.
9. Support in tracking and reporting healthcare activities, including screenings and case management.
10. Perform other administrative tasks as needed.

Requirements:

1. Diploma or Degree in relevant fields such as Social Work, Nursing, Psychology, or Allied Health.
2. Strong communication and interpersonal skills with a service-oriented mindset.
3. Prior experience in customer service, healthcare, or social services is preferred.
4. Ability to work independently and as part of a team in a fast-paced environment.
5. Proficient in IT skills and attention to detail.
