The Manager is responsible for planning, managing, and monitoring the care integration projects driven by Primary Care & Clinical Networks.
The job duties include the following:
Work with the Director and Senior Manager to propose and seek continuous funding support from MOH/AIC.
Provide operational oversight and drive implementation of operational plans.
Monitor the utilization of programme resources (manpower and funding).
Liaise and communicate effectively with the different stakeholders on the projects.
Coordinate and provide secretarial support for SORH meetings.
Adhere to timelines and monitor the progress of the projects assigned.
Supervise and provide oversight to the GP Engagement Team.
Manage and handle service recovery issues faced.
Assist the Assistant Director in identifying and screening new GP Partners to assess their suitability and alignment of interests.
Ensure close partnership with DOT GP Partners.
Work closely with GP Ambassadors to ensure that DOT GPs are serviced professionally and adequately.
Derive training curriculum that includes both scenario-based practice as well as on-job training.
Screen and recruit like-minded doctors to partner in various programmes run by the department.
Conduct periodic clinic visits to maintain relationships with existing GP partners.
Establish long-term partnerships with vendors such as respective SmartCMS providers to form a strong community of support to better support the DOT PCN clinics.
Oversee the organizing and execution of various GP events that target between 100 to 250 GPs at each event.
Support the institution’s effort to devise and implement Healthier SG initiatives.
Work with internal stakeholders across departments to explore new initiatives that can better support GP partners.
Facilitate the negotiation of contractual terms and drafting of legal agreements between SingHealth and all collaborative partners and service providers (i.e., GP clinics, ancillary service providers, and private laboratory vendors).
Provide quarterly data to MOH/AIC on PCN related clinical outcomes and other indicators.
Conduct market surveys to better understand the needs of GPs and develop the PCN to include value propositions that incentivize GPs to provide better management of care to their patients.
Any other ad-hoc matters as instructed by the manager.
Job Requirements:
Bachelor's Degree in Life Sciences, Business Administration, Political Sciences, or Humanities with at least 8 years of working experience would be an advantage.
Strong managerial skills and ability to lead and manage large teams.
Experience in partnerships and stakeholder management.
Strong communication skills.
Experience in healthcare would be an added advantage.
Proficient in using Microsoft Office (Word, Excel, and PowerPoint).
