Job description

Job Summary:

Care Coordinator is responsible to work in collaboration with multi-disciplinary health care teams/ community service providers to provide coordination and continuity of patient care across the healthcare continuum; to support patients and their family members to transit care from hospital to home and in the community.

Responsibilities and Duties:

  1. Participate in discharge planning and identify patient requiring post discharge care transition support.
  2. Plan and carry out post discharge support and follow up via phone call and/or home visit to ensure patients and caregivers ability to cope at home and discharge care plan is implemented.
  3. Assist patient and their families to streamline and coordinate the range of community service available.
  4. Implement timely and appropriate care coordination and evaluate the outcome accordingly.
  5. Act as liaison person, collaborating with patients, families and the multidisciplinary teams to achieve best outcome for the patients.
  6. Monitor patients general medical condition during home visit and report to clinician if necessary.
  7. Promote positive changes in patients lifestyle in the community.
  8. Help patients and their families to plan for and improve end of life care.
  9. Participate in hospital activities that contribute towards improving the quality of patient care such as research, quality or innovative projects that enhance patient care.
  10. Support data collection and preparation of reports and statistics for outcome tracking.

Job Requirements:

  1. Possess diploma/degree or equivalent professional qualifications in Nursing.
  2. At least 3-5 years of experience in nursing or related field in acute and/or community settings in Singapore.
  3. Understanding of Singapore healthcare system, community services and resources of patient post discharge.
  4. Experience in care coordination or case management is highly valued.
  5. A critical thinker, self-motivated, able to work collaboratively with multi-disciplined team.
  6. Excellent interpersonal and communication skills.
  7. Able to speak local languages and dialects will be an advantage.
  8. Familiar with MS Word, Excel, and PowerPoint.
