Conduct accurate and holistic speech and language specific assessment for clients during admission, periodic care plan reviews, and discharge.
Select appropriate and specific outcome measures to enable evaluation of therapy progress and outcomes in a timely manner to guide clinical decision making.
Communicate assessment results, intervention plans, goals, outcomes, and discharge plans to clients and relevant stakeholders.
Formulate discharge and continuity of care plans based on clients’ performance in consultation with relevant stakeholders.
Facilitate community integration through collaboration with other community partners as required.
Prescribe assistive devices and equipment according to clients’ needs.
Mentor and supervise therapists and support staff for professional development and day-to-day job scope.
Plan induction and clinical orientation for new team members.
Identify safety risks and issues, recommend preventive and corrective care measures.
Oversee inventory management and equipment maintenance required for therapy services.
Conduct service and clinical audits, and review audit findings at team level.
Requirements
Bachelor / Master Degree in Speech Pathology or its equivalent.
Must be fully registered with AHPC.
