Assist/ Care Coordinator

As part of a multidisciplinary team, you will help deliver comprehensive care, empowering patients and caregivers with technology to manage their health. You’ll also raise awareness of available resources and encourage proactive health actions.

Key responsibilities include:

• Building relationships with internal and external stakeholders.
• Supporting clinical goals like preventive screenings, reducing readmissions, and gathering social health data to improve outcomes.
• Using technology to improve patient health literacy and access to resources.
• Assisting in patient care coordination and documenting clinical indicators.
• Performing basic health assessments and closing gaps in screenings and vaccinations.
• Providing health education on lifestyle management, physical activity, and advising on health monitoring devices and vaccinations.

Requirements:

• Diploma preferred.
• Preferably with some working experience in healthcare.
• Good verbal and written communication skills.
• Possess good interpersonal skills, a positive attitude towards work, and be a team player.
