Adopts a patient-centered approach and principles of the social determinants of health for patient management.
Provides nutrition assessment, nutrition support, counselling and education to patients and carers, individually or in groups (specialised discipline).
Provides nutrition intervention based on various therapeutic diets through different modes of communication (e.g., phone/virtual/home visit/centre visit consultation).
Reviews nutrition progress and documents patient management (specialised discipline).
Participates in multidisciplinary team discussion for patient care.
Organises and participates in delivery of nutrition education programmes for patients, carers, public, community providers and healthcare professionals.
Acts as a mentor for junior Dietitians on issues regarding nutrition and Dietetics when applicable.
