Job Description

You will be responsible to provide a high standard of care through care management, assessment, planning, implementation, and evaluation of nursing care to meet the needs of the clients, including educating family members.

You will provide cost-effective care in accordance with the established policies and procedures, standards of nursing care, and practices.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

Primary Responsibilities and Duties (80%)

  1. Complies with approved standards of nursing practices.
  2. Responsible for assessing the needs and delivery of quality geriatric / dementia nursing care to clients.
  3. Perform pre-assessment of new clients referred by AIC.
  4. Ensure the clients’ enrolment complies with the Company’s SOP.
  5. Administer the documentation to be submitted to the relevant government body.
  6. Assess and coordinate with the health care team to assess, plan, implement and evaluate client’s conditions.
  7. Monitor, assess and review clients’ progression and implement care plan.
  8. Provide health promotion and counselling to clients and families.
  9. Consult and coordinate with the health care team to assess, plan, implement and evaluate client’s conditions.
  10. Perform nursing care such as medication administration, wound dressing, and other technical skills as required.
  11. Oversee and manage the performances of the nursing team / care staff to ensure the desired level of work competency.
  12. Coordinate, implement, and monitor clinical and service quality performance indicators for the continuous improvement plan.
  13. Perform CPR, AED, and first aid to clients and play an active role in emergency situations.
  14. Assist and support the delivery of health promotion activities in the Centre.

Secondary Responsibilities and Duties (20%)

  1. Participates in clinical service projects.
  2. Participates in infection control campaign activities.
  3. Assumes additional responsibilities as assigned by Senior Nurse Manager.
  4. Participates in training and development of care staff.

Additional Responsibilities:

  1. Demonstrate accountability for one’s development and status as a professional Registered Nurse.
  2. Other duties as assigned by the SNM / Center In Charge.
Qualifications
  1. Registered Nurse with Singapore Nursing Board.
  2. Degree or Diploma in Nursing.
  3. Advanced / Specialist Diploma in Geriatric / Community Nursing is an added advantage.
  4. Minimum 3 years of relevant work experience is preferred.
