Clinical Instructor (STAFF NURSE/COMMUNITY HOSP) - up to $6k/Central/Office Hours #HAO

Salary: up to $6000

Working Days & Hours: Mon-Fri Office Hours

Job Description:

1. Implements patient care in collaboration with the multi-disciplinary teams (MDT) and according to assessed needs and health condition(s).
2. Facilitates admissions, transfers and discharges; ensures discharge planning is done timely upon patient’s admission and appropriate to condition.
3. Follows up and evaluates patients’ progress according to planned care and readiness for discharge.
4. Collaborates with the MDT to deliver evidence-based care to patients to achieve the desired care outcomes.
5. Monitors patient for changes in condition, recognises early signs of clinical deterioration using appropriate physiological assessments and escalates as required to enable timely implementation of interventions.
6. Participates and contributes actively in MDT’ rounds, executes physicians’ treatment and investigation orders promptly.
7. Prepares and assists physicians with bedside patient procedures and treatments.
8. Communicates and discusses patients’ progress and response to treatment in collaboration with the MDT.
9. Demonstrates clinical skills, knowledge, critical thinking and reasoning in the delivery of patient care.
10. Assists nursing team to problem solve if there are any patient/clinical/or operational issues and seeks appropriate assistance/escalates to Senior Nurse Manager (SNM)/Nurse Manager (NM).
11. Delivers patient and/or family education on disease progression, prevention and rehabilitation and assesses understanding of condition to enable participation in self-health care management.
12. Assists department supervisors to minimise use of resources keeping expenses within approved budget.
13. Reviews infection control practices, monitors compliance and identifies areas of improvement.
14. Plans and implements in-services for nurses to extend skills and knowledge to maintain safe and quality nursing care.
15. Supports new initiatives, leads changes implemented by Nursing in collaboration with other stakeholders.
16. Reviews clinical performance of new nurses and nursing and provides constructive feedback and documentation to appropriate personnel.
17. Acts as a competent clinical resource and supervises the usage of medical equipment; Electronic Medical Records (EMR) and other electronic systems; clinical workflow and processes; co-leading in clinical emergencies or crisis situations.

Job Requirements: Min Degree in Nursing w min 5 yrs of relevant perm inpatient & senior nursing experience, preferably in gerontology/palliative care w Advanced / Specialist Diploma in Geriatric / Community Nursing & Long-Term Care settings.
