Our Client, a leading and well-known Health Institution in Singapore is inviting qualified candidates to fill the position as Clinic Care Coordinator.

Job Responsibilities

• Assisting in coordinating and providing patient care activities.
• Using IT systems to track, perform and document established clinical indicators and care given to patients.
• Performing basic parameter measurements and closing preventive screening and vaccination gaps in relation to national programmes/initiatives.
• Providing basic health education to improve patients’ knowledge on lifestyle management, social and physical activities through social prescribing, and advising patients on vaccination requirements.

Job Requirements

• Diploma in Health Promotion will be an advantage.
• Prior healthcare experience will be advantageous.
• Good verbal and written communication skills.
• A team player with good interpersonal & communication skills.
• Passionate to work in the Healthcare industry.
• Customer-oriented, a team player, a fast learner, and serious about work.
