Our Client, a leading and well-known Health Institution in Singapore is inviting qualified candidates to fill the position as Clinic Care Coordinator.

Job Responsibilities

• Assisting in coordinating and providing patient care activities.
• Using IT systems to track, perform, and document established clinical indicators and care given to patients.
• Performing basic parameter measurements, closing preventive screening and vaccination gaps in relation to national programmes/initiatives.
• Providing basic health education to improve patients’ knowledge on lifestyle management, social and physical activities through social prescribing, the use of health monitoring devices e.g. home blood pressure, glucose and weight monitoring, as well as advising patients on vaccination requirements.

Job Requirements

• Diploma in Health Promotion will be an advantage.
• Prior healthcare experience will be advantageous.
• Good verbal and written communication skills.
• A team player with good interpersonal & communication skills.
• Passionate to work in the Healthcare industry.
• Customer-oriented, a team player, a fast learner, and serious about work.
