- Organise outreach events (in-house or with partners) to residents in Active Ageing Centre (AAC) service boundary with an aim to assess seniors using the Community Screener Tool to identify at-risk individuals.
- Organise programmes (e.g., activities, functional screening) to improve health outcomes - physical health, mental wellness, and social capital.
- Use the Asset-based Community Development approach to recruit and activate the community seniors.
- Support seniors to plan, coordinate, and activate their neighbours in championing and participating in AAC programmes.
- Manage befriending and buddying programme to isolated seniors with proper monitoring and record keeping.
Care Referral and Relationship Building
- Network with seniors and community partners.
- Manage donations, volunteers, and community partners as needed.
- Maintain AAC volunteer database and conduct volunteer orientation and training.
- Build up network with community agencies (e.g., Silver Generation Office, Social Service Office) to provide seamless referral of services for seniors.
- Refer seniors (where applicable) to HNF home care services, social welfare department for continuum of care.
Administration & Facilities
- Lead administration and operations in AAC to ensure smooth operations and environmental safety.
- Assist to train and manage a team of staff in AAC to maintain a safe and motivating workplace.
- Assist Centre Manager with recruitment, hiring, and training.
- Assist in the development of standard operating procedures, contingency, and emergency planning.
- Assist the Centre Manager in managing adverse incidents, enquiries, and feedback.
- Monitor centre statistics (attendance, submission of reports on attendance to programmes, befriending statistics) to management and authorities (e.g., AIC/MOH).
- Ensure timely and accurate submission of reports, claims, documentation, or statistics to stakeholders.
- Manage audits and investigations, record and write minutes for the centre's meeting when required.
- Review AAC policies and processes when required.
- Ensure cost-effectiveness of centre operations, coordinate facility repairs, maintenance of equipment, and related documentation in a timely manner. Purchasing of centre supplies, handling suppliers, vendors, and contractors.
- Provide cross coverage to colleagues or deployment to other centres when need arises.
Qualifications:
- Diploma or Degree in Social Work, Nursing, or Healthcare-related field with at least 3 years of experience in senior, social, or health care setting.
- Engaging with good interpersonal communication, facilitation, and relationship-building skills.
- Patient, approachable, and enjoy working with seniors, family members, and peers of different age groups.
- Strong work ethic, reliable, able to handle stress with a pleasant disposition and positive attitude.
- Willing to work staggered shifts according to Centre operating hours and overtime when required.
Working Hours: Mon to Thurs: 8am to 5.30pm, Fri: 7.30am to 5pm
