Our established client in the healthcare industry is looking for a Case Management Officer!

Responsibilities:

• Assess, plan, implement, and evaluate care for patients with renal disease.
• Provide patient education and counseling to patients and caregivers.
• Assist in coordinating renal replacement therapy and placement in dialysis facilities.
• Support and co-lead specialty nurse-led clinics for patient care.
• Work closely with doctors, nurses, and paramedical staff to enhance patient care.
• Ensure proper use and maintenance of medical equipment and supplies.
• Review and develop patient education materials and clinical pathways.
• Mentor newly joined renal coordinators and participate in quality improvement activities.
• Maintain a clean and safe working environment in compliance with healthcare standards.

Requirements:

• Minimum Degree in Nursing, Biomedical Science, Life Sciences, Biotechnology, or General Science.
• Registered Nurse with at least 3 years of clinical experience.
• Strong clinical skills, critical thinking, and patient care knowledge.
• Excellent communication and interpersonal skills.
• Proficiency in Microsoft Office (Word, Excel, PowerPoint)
