Case Management Officer, Cardiac Rehab Patient (Nurses Move Out of Clinical) #HJC

Job Purpose

The case management officer reviews patients (inpatient and outpatient) and assesses suitability for enrolment under the Cardiac Rehabilitation Programme (CRP). These include assessing, coordinating and implementing care, teaching, and counselling plans to help patients and their families with more complex needs adapt to changes.

The case management officer is also responsible for clinical practice and patient education in the nursing specialty/subspecialty.

Primary Responsibilities

Inpatient CRP (Cardiac Wards)

  1. Identify patients through proactive screening daily.
  2. Assess, plan, implement, and evaluate, conduct counselling and teaching for patients and their families/caregivers.
  3. Provide patient/family information and education on disease processes, causes of disease conditions, health education on risk factors, nutritional counselling, smoking cessation advice, exercise & coping strategies at home following discharge.
  4. Ongoing collaboration among patient, family, and a multidisciplinary team to provide valuable connections.
  5. Refer patients to various needs or departments accordingly (e.g., Dietician, Physiotherapist, Smoking Cessation Counsellor).
  6. Screen, coordinate selection, and recruitment of patients into the outpatient Cardiac Rehabilitation Programme (CRP).
  7. Schedule outpatient Pre-Rehab Assessment Clinic follow-up appointments when appropriate.
  8. Triage the referral, contact patient for more information, ensure correct referral to appropriate exercise rehab program with sufficient time allocation for patient’s visit to respective physician/therapist appointment booking.
  9. Check and highlight clinic/CRP orientation appointment list for enrolled patients.
  10. Coordinate and assist in Pre-Rehab Assessment clinic and schedule Post Rehab Clinic at the end of the program with post-program parameters.
  11. Input the information into the Rehab Clinic database.
  12. Assume responsibility for data collection and entry into CRP database system.
  13. Coordinate patients’ access and treatment sequence where required for the specialty.
  14. Tele-care to reschedule patient’s appointment date when defaulted outpatient follow-up.

Outpatient CRP (Heart Rehab) – Clinical

  1. Referral can be from heart clinics, wards, self-referral, etc.
  2. Triage the referral, contact patient for more information, ensure correct referral to appropriate exercise rehab program with sufficient time allocation for patient’s visit to respective physician/therapist appointment booking.
  3. Check and highlight clinic/CRP orientation appointment list for enrolled patients.
  4. Coordinate and assist in Pre-Rehab Assessment clinic and schedule Post Rehab Clinic at the end of the program with post-program parameters.
  5. Input the information into the Rehab Clinic database.
  6. Assume responsibility for data collection and entry into CRP database system.

Outpatient CRP (Heart Rehab) – Patient Education

  1. Conduct nurse-led specialty education sessions for patients and their families.
  2. Plan, implement, and evaluate in-service talks and specialty training courses based on unit and individual training needs.
  3. Ensure patient education and counselling is carried out according to established standards of care and practice using the approved teaching materials.
  4. Apply understanding of prevention of medico-legal issues in relation to patient education.
  5. Assist to review and develop patient education materials in consultation with doctors and paramedical staff.

Requirements

  1. Any science-related Degree.
  2. Registered Nurse with at least 3 to 5 years of clinical experience in Acute/Cardiac Nursing, of which a minimum of 2 years must be spent in the relevant specialty and as a Staff Nurse II is preferred.
  3. A critical thinker, self-motivated, and able to work as part of a multi-disciplinary team.
  4. Good interpersonal and communication skills.
  5. Ability to use local languages and dialects.
  6. Competent in IT skills (Microsoft Word / PowerPoint).
