Case Management Officer (Cardiac Rehab/ 5 days office hours)

Job scope:

  1. Identify patients through proactive screening daily.
  2. Assesses, plans, implements and evaluates, conducts counselling and teaching for patients and their families/caregiver.
  3. Provides patient/family information and education on disease process, causes of disease conditions, health education on risk factors, nutritional counselling, smoking cessation advice, exercise & coping strategies at home following discharge.
  4. Ongoing collaboration among patient, family and a multidisciplinary team to provide valuable connections.
  5. Refer patients to various need or department accordingly (e.g. Dietician/ Physiotherapist/ Smoking Cessation Counsellor).
  6. Screen, coordinate selection and recruitment of patient into the outpatient Cardiac Rehabilitation Programme (CRP).
  7. Schedules outpatient Pre-Rehab Assessment Clinic follow-up appointments when appropriate.
  8. Triage the referral, contact patient for more information, ensure correct referral to appropriate exercise rehab program with sufficient time allocation for patient’s visit to respectively physician/therapist appointment booking.
  9. Check and highlight clinic / CRP orientation appointment list for enrolled patients.
  10. Coordinate and assist in Pre-Rehab Assessment clinic and schedule Post Rehab Clinic at the end of the program with post program parameter.
  11. Input the information into the Rehab Clinic database.
  12. Assume responsibility for data collection and entry into CRP database system.
  13. Coordinate patients’ access and treatment sequence where required for the specialty.
  14. Tele-care to reschedule patient’s appointment date when defaulted outpatient follow up.
  15. Conducts nurse-led specialty education session for patients and their families.
  16. Plans, implements, and evaluates in-service talks and specialty training courses based on unit and individual training needs.
  17. Ensures patient education and counselling is carried out according to established standards of care and practice using the approved teaching materials.
  18. Applies understanding of prevention of medico-legal issues in relation to patient education.
  19. Assists to review and develop patient education materials in consultation with doctors and paramedical staff.
  20. Ensure daily operations of these service points, including administrative/operational matters and partnering Clinic Manager to solve system and process problems.
  21. Handle purchase, work requisition processes and calling suppliers for quotation.
  22. Ensure sufficient supply of related educational material / stationery requisition and other equipment maintenance are maintained.
  23. Updates and maintains patient consultation/CRP training statistics and CRP support group.

Requirements:

  1. Any science related Degree.
  2. Registered Nurse with at least 3 to 5 years of clinical experience in Acute / Cardiac Nursing, of which a minimum of 2 years must be spent in the relevant specialty.
