A healthcare organization providing innovative programmes, support services, education and training in support of stroke survivors, their caregivers, volunteers, professionals, and the public is looking for a Care Coordinator.
JOB RESPONSIBILITIES:
Advising clients on financial scheme applications, ensuring timely updates of renewal of means testing and financial assistance.
Managing the onboarding process for new clients, assisting in appointment scheduling/rescheduling, subsidies application, and addressing client inquiries for rehabilitation.
Assisting clients/caregivers with appropriate information and referral assistance.
Conducting joint home visits with a case manager/social worker or the multidisciplinary team.
Consolidating, preparing, and tracking relevant monthly reports and statistics.
JOB REQUIREMENTS:
Minimum Diploma, preferably in a healthcare discipline; fresh graduates are welcome to apply.
Relevant prior experience as a Care Coordinator/social work assistant/social work associate is an added advantage.
A diploma in Social Work might be considered for a senior care coordinator role.
