Care Coordinator (Public Healthcare)

Job Description:

• Work closely with nurses to identify patients requiring counselling and support
• Collaborate with clinics to obtain informed consent for patient enrollment, referrals and care coordination process within the primary care network
• Liaise with the service providers in scheduling ancillary service deployments
• Prepare the report after each deployment
• Track and submit monthly ancillary report
• Provide administrative support to the admin team in administering surveys and collating data for analysis and reporting
• Any other duties as per assigned by supervisor

Requirement:

• Diploma in Healthcare Management/ Counselling/ Social Work/ Business Management or equivalent
