Job Description:

• Evaluate, plan and implement care for chronic kidney disease patients
• Conduct teaching and counselling for patients and patient family
• Collaborate with Allied Health team and physicians in the delivery of patient care and service
• Maintain related database
• Provide patient's report to the Programme Manager or Doctor where required
• Any Ad hoc duties as per assigned

Requirement:

• Bachelor Degree in Life Science, Nursing, Medical Sciences or related medical fields
