Care Coordinator (Community Settings)

As a Care Coordinator, you will be responsible for the following duties:

• Support clinical team in assessments, case work, and programs to resolve clients' personal, social, emotional, and financial issues.
• Provide service coordination management, administrative support, and operational improvements.
• Collaborate with colleagues across departments for a seamless client experience.
• Assist in advising clients on financial scheme applications and ensure timely updates of means testing and financial assistance renewals.
• Support administrative processes for delivering services to clients and caregivers.
• Manage onboarding for new clients, including scheduling/rescheduling appointments, subsidy applications, and addressing rehabilitation inquiries.
• Assist in the initial assessment of client/caregiver needs with basic case management under guidance of case manager/social worker.
• Help clients and caregivers access community services to improve coping abilities.
• Provide information and referral assistance to clients and caregivers.
• Conduct joint home visits with case managers/social workers or multidisciplinary team as needed.
• Support events and projects related to client and caregiver support.
• Participate in case discussions with the clinical team.
• Consolidate, prepare, and track monthly reports and statistics.
• Ensure timely and accurate documentation in systems and paper filing.

Requirements:

• A Diploma in Social Work or any relevant qualification, along with experience in care coordination or as a social work associate.
