About the Role
We are seeking Care Coordinators to join a multidisciplinary healthcare team in one of our Public Healthcare Instituition. This role is ideal for individuals passionate about empowering patients and caregivers to take charge of their health through technology and holistic care.

Key Responsibilities:

• Work closely with the care team to deliver personalized and holistic healthcare solutions.
• Develop and sustain strong relationships with both internal and external stakeholders, including patients and caregivers.
• Contribute to key strategic objectives such as increasing preventive health screening participation and reducing hospital readmission rates.
• Efficiently utilize IT systems to document care activities and track clinical performance indicators.
• Conduct basic health assessments and address gaps in preventive screenings and immunizations.
• Educate patients on lifestyle modifications, the use of health monitoring tools, and vaccination guidelines.
• Utilize technology to improve patients’ health literacy and facilitate access to healthcare resources.

Job Requirements:

• Diploma or higher educational qualification.
• Previous experience in healthcare is beneficial.
• Strong proficiency in English, with additional fluency in Mandarin or another language considered an advantage.
• Competence in Microsoft Office applications (Excel, Word, PowerPoint) and the ability to navigate IT systems effectively.
• A friendly demeanor, excellent interpersonal skills, and a positive mindset.
• Self-driven, collaborative, and deeply committed to patient care.
