Job description

Responsibilities

  1. Plan, implement and monitor the care process, referral and follow-up workflow.
  2. Provide supervision to the team to provide care and support, in alignment with the overall programme objectives, adhering to the relevant ethical and legal guidelines.
  3. Provide care coordination and service linkage to clients with the appropriate resources and support, including caregiver support.
  4. Responsible for establishing the network with other stakeholders like community partners and/or related agencies to build collaborative partnerships and better facilitate care and support for clients and caregivers.
  5. Establish and review work and communications workflow including identification, referral and escalation work processes as well as the development/update of the service manual.
  6. In charge of the analysis and review of the collated data, and monitoring of the team's progress and performance.
  7. Responsible for ensuring that the claims and data submission to AIC is timely and accurate.
  8. Ensure programme data and documents are kept and maintained, in accordance with the programme requirement, as well as the prevailing personal data protection and security guidelines.
  9. Oversee the training and professional development of the team with relevant competencies to provide the appropriate care and support for clients and caregivers, aligning to the intended care model.
  10. Other ad-hoc assignments at the discretion of the Department Head.

Qualifications

  • Bachelor's Degree preferably in Social Work/Psychology/Sociology/Gerontology/Counselling.
  • Minimum 3 years' experience in community healthcare settings in Singapore.
  • Good knowledge of geriatrics and community services.
  • Proficiency in Google Suite applications.
  • Good communication skills (oral and written).
  • Passion in community services and service oriented.
  • Good judgement and decision making skills.
  • Good in problem solving.
  • Good record keeping abilities and attention for details.
  • Possess 1-2 years of experience in administration.
  • Meticulous and independent.

Other Information

  • Ability to speak English, Mandarin and Malay to communicate with clients that are only proficient in one language would be advantageous.
