Job Responsibilities

1. Provide case management via a coordinated process of care planning and coordination.
2. Conduct post-discharge phone call follow-up and telehealth monitoring under the supervision of a Care Coordinator.
3. Work with Care Coordinators and nurses to develop and manage care for patients or residents in the community.
4. Act as a health coach and provide health screening for residents in the community and health post.

Job Requirements

• Possess a diploma in any discipline (healthcare-related diploma will be an advantage).
• Prior relevant work experience will be an advantage. Entry-level candidates are welcome to apply.
• Good knowledge of various community services and support in the community setting will be an advantage.
• Able to work as a team.
• Strong organisational, coordination, interpersonal, and communication skills.
• Proficient in computer skills and Microsoft Office.
• Able to converse in local languages is preferred.
• Able to work on weekends (on a rotation basis).
