Allied Health Professional/Associate Psychologist (COMIT) (Hybrid Arrangement)

Responsibilities

• Conduct case assessment and formulate Individualised Care Plans for Persons with Dementia and their carers.
• Provide interventions such as psychoeducation, supportive counselling, and service referral. Able to administer Advance Care Planning will be a bonus.
• Liaise and collaborate with multidisciplinary professionals and external service providers for holistic care management.
• Provide case recordings and relevant documentation.
• Participate in team meetings and case conferences as required.
• Assist in the preparation of data and statistics for programme evaluation and reporting purposes.
• Plan and facilitate Caregiver Support Group sessions.
• Support in outreach events and talks.
• Any ad-hoc tasks as and when required.

Requirements

• Preferably with a degree in an allied health profession/social work/ psychology etc.
• Good oral, written and interpersonal communication skills.
• The ability to facilitate group sessions will be an added advantage.
• Able to work on occasional Saturdays and weekday evenings.
• Passionate about dementia care and working with people.
• Prior working experience in dementia care will be an added advantage.
