Job Description

The General Practitioner (GP) in Home Healthcare will provide comprehensive medical care to patients in their homes. This role involves conducting home visits, developing management plans, reviewing cases, and ensuring the highest standards of clinical quality and infection control practices. The GP will work closely with a multidisciplinary team to deliver effective and compassionate care to patients in their home environment. Will also co-ordinate comprehensive patient care for individuals receiving medical services at home. Will assess patient needs, develop care plans, and collaborate with other healthcare providers, caregivers, and community resources to ensure quality and continuity of care. This role is crucial in supporting patients with chronic illnesses, disabilities, or post-hospitalization recovery.

Patient Assessment and Care Planning

  • Conduct comprehensive patient assessments (patient intake) of patients' medical and social needs and identifying risks, barriers to patient care.
  • Develop individualized care plans in collaboration with specialized physicians, nurses, and healthcare providers.
  • Utilize clinical skills to evaluate patients' health conditions and needs in their home settings.

Case Reviews, Consultations and Development of Management Plans

  • Review ongoing home healthcare cases and prepare summaries for the consultant in charge.
  • Create individualized management plans for patients based on assessments with supervision of HHC Medical Lead.
  • Collaborate with patients and families to discuss treatment options and establish goals.

Co-ordination of Healthcare Services

  • Work collaboratively with healthcare professionals from various disciplines.
  • Participate in team meetings and coordinate care plans with nurses, therapists, and social workers.
  • Identify home medical needs by arranging and facilitating appointments, therapy sessions, and home health services while ensuring seamless communication between healthcare providers, patients, and caregivers.

Monitoring and Follow Up

  • Conduct follow-up visits and phone check-ins to assess patient progress, adjust care plans based on changes in patient conditions or healthcare needs, monitor medication adherence, and facilitate prescription refills as needed.

Clinical Quality Assurance

  • Ensure the clinical quality of services provided to patients in their homes.
  • Implement best practices and adhere to clinical guidelines and protocols during home visits.
  • Maintain accurate and timely documentation of all patient interactions and services while ensuring compliance with healthcare regulations and organizational policies; participate in audits, quality improvement initiatives, and case reviews.
  • Implement and monitor infection control practices in home healthcare settings.
  • Educate patients and families on infection control measures and ensure compliance during visits.

Patient and Caregiver Education

  • Educate patients and caregivers on disease management, treatment plans, and self-care practices while providing guidance on available social services (if available) and addressing concerns and questions related to treatment plans.

Skills

  • Excellent communication, problem-solving, and advocacy skills.
  • Ability to work in a team and manage a caseload effectively.
  • Familiarity with home healthcare practices and regulations.
  • Proficiency in using Healthcare Information Systems (HIS).
  • Valid SCFHS licensure.
  • Strong knowledge of chronic disease management and home healthcare services.

Education

  • Bachelors Degree in Medicine M.B.B.S.
