We are looking for an exceptional Home Care Physical Therapist to join our team and make a real difference in the lives of those recovering from illness and injury. Work primarily weekdays with a weekend rotation monthly. Enjoy the autonomy of managing your daily work schedule.
About the Role:
As a Home Care Physical Therapist, you will:
- Provide both direct and indirect patient care to facilitate the rehabilitation and restoration of normal body functions.
- Implement personalized treatment programs based on each patient's plan of care.
- Administer therapeutic modalities and train patients in therapeutic exercises, transfers, and gait.
- Supervise and guide PT Assistants and PT Aides to ensure the highest quality of care.
Responsibilities
- Physical Therapy Assessment and Skilled Treatments.
- Client Care Coordination.
- Teaching, Supervision and Counseling Clients and Caregivers.
- Documentation of Interdisciplinary Communication and Supervision of Ancillary staff.
- Problem Solving and Critical Thinking Skills.
- Ability to effectively and safely transfer patients with significant physical and/or mental impairment.
- Time Management and Development of Plan of Care.
Qualifications
Ability to assist in patient lifts, transfers, and guarded functional activity training.
Minimum required: Bachelor's degree.
Preferred/desired: Master's degree or doctorate.
Competencies and Skills
- Clear Communication Skills (Both Written And Verbal).
- Able To Keep Confidential Information Regarding Patients and Team Members.
- Able To Withstand Crisis Situations.
- Skills to Provide Customer Service To Patients, Team Members and Visitors.
- Proof Of Current/Valid Car Insurance.
- Knowledge And Experience With Electronic Health Records.
- Experience With Excel, Power Point, Word, Visio, Etc.
- Valid Driver's License With No More Than 3 Moving Violations.
Credentials
Essential: PT - Physical Therapist, BLS - Basic Life Saving certification.
Education
Essential: Bachelor of Science (BS).

Expert: USD $60.16/Hr.
