Responsible for performing various functions through collaborative drug therapy management and patient care including day-to-day operations and other clinical services related, as well as overseeing Retail Pharmacy and assisting the Head of Pharmacy in the overall management of the Hospital Pharmacy in terms of effective and efficient operation, cost-effective supply, and safe medication practice.
Job Responsibilities:
Medication Management:
1. Ability to evaluate the appropriateness of medications therapy and dosage according to patient's diagnosis.
2. Keep proper documentation of medications plans, patient progress, and results of pharmaceutical treatment.
3. Ensure the selection, preparation, and dispensing of medications, including chemotherapy agents, biologics, and supportive care drugs.
4. Responsible for the management and safe handling of radioactive materials and radiopharmaceuticals used in diagnostic imaging and therapeutic procedures.
5. Perform medication dispensing and medication counselling.
6. Obtain and disseminate necessary drug or drug therapy related information.
Cytotoxic Drug Reconstitution:
7. Review, interpret, and verify oncology prescriptions for accuracy, appropriateness, and adherence to treatment plans.
8. Counter check final CDR product release.
Inventory Control/Management:
9. Perform other related duties as assigned and directed by the immediate superior.
10. Be the person in charge of scheduling for Pharmacists and Pharmacy Technicians in the service areas.
11. Organize the day-to-day operations in the designated areas of services.
12. Ensure that only approved products are stored or available for use in the hospital.
13. Ensure the availability of formulary drugs at all service areas.
14. Ensure proper inventory management of pharmaceutical products through periodic cycle count at service areas and drug checks at all storage locations.
15. Ensure the security of dangerous drugs, psychotropic substances, and controlled drugs are maintained at all times and its inventory is properly managed.
16. Participate, monitor, assess, and review departmental KPI or quality indicators.
17. Undertake any other duties as assigned by the Head of Pharmacy.
Research and Clinical Trials:
18. Support oncology research and clinical trials conducted within the hospital by participating in the preparation and administration of investigational drugs.
19. Maintain accurate records and documentation of clinical trial-related activities.
20. Collaborate with medical and healthcare teams to ensure accurate diagnosis and effective treatment of patients.
21. Contribute to a continuous learning environment and innovation within the organization.
22. Supervise clinical activities offered by the department and re-evaluate periodically to ensure that clinical resources are most appropriately used.
Medication Safety and Quality Assurance:
23. Develop clinical pharmacy programs according to policies & procedures.
24. Take reasonable care at the workplace for the safety of oneself and others and comply with any instruction or measures on occupational safety, health, and environment as required.
25. Report all accidents/incidents and hazards to the superior and complete the Incident Report Form for each accident/incident.
26. Ensure all documents required by legislation are preserved and stored appropriately.
Training & Supervision:
27. Carry out orientation programs for new staff in the department.
28. Perform periodic evaluation of respective staff in the department.
29. Develop and implement training programs for the departmental personnel.
30. Participate in the development and implementation of training programs in pharmacy areas for pharmacy staff.
