Cytotoxic Drug Reconstitution (CDR) Pharmacist

Generally responsible for the safe usage and distribution of drugs in the hospital to ensure all drugs are supplied and distributed efficiently and accurately in a timely manner to all in-patients and out-patients.

• Check and Supply of Ward stocks, Dangerous Drugs and Psychotropic Substances to internal departments.
• Ensure medications are stored and dispensed appropriately to maintain the efficacy and quality.
• Adhere and prepare formulation in accordance to Good Manufacturing Practice.
• Prepare cytotoxic medications under aseptic conditions in accordance to policies and procedures.
• Check quality and/or sterility of final product manufactured is in accordance to prescription or formulation.
• Adhere to the requirements of maintaining clean room and manufacturing room.
• Ensure clinical appropriateness and monitoring of drug treatment e.g. efficacy, adverse effects, drug interactions, compatibility.
• Provide quality service of medication history taking, reconciliation, and counselling.
• Coordinate efficient discharge planning (e.g. timely organization of ‘drugs to take home’, advise on drug administration via enteral feeding tubes at home, discharge counselling).
• Participate in clinical related quality improvement projects.
• Participate in evaluation and sharing of new drug requests (NDR).
• Manage day to day Cytotoxic Drug Reconstitution (CDR) and Manufacturing operations.

Job Requirements

• Degree in Pharmacy recognized by Pharmacy Board of Malaysia.
• Registered Pharmacist with the Pharmacy Board of Malaysia with current Annual Retention Certificate and valid Type A license (applicable upon joining).
• Working knowledge of Poison Act 1952, Dangerous Drugs Act 1952, Sale of Drugs Act 1952, Registration of Pharmacist Act 1951 and Medicines (Advertisement and Sale) Act 1956, and the regulations, other healthcare related Acts, regulations and accreditation standards.
• 3 years of working experience as a pharmacist with 1-2 years of working experience as a CDR pharmacist.
