Case Manager

About the role

Llayana Clinic is seeking a dedicated Case Manager to join our team in our Kuala Lumpur City Centre location. As a Case Manager, you will play a vital role in coordinating and overseeing the care and support provided to our patients, ensuring they receive the highest quality of service. This is a full-time position that will allow you to make a meaningful impact on the lives of those we serve.

What you'll be doing

1. Assessing patient needs and developing comprehensive care plans to address their unique requirements
2. Coordinating with healthcare providers, social services, and other stakeholders to ensure seamless delivery of care
3. Monitoring patient progress and adjusting care plans as needed to optimise outcomes
4. Advocating for patient rights and serving as a liaison between patients, families, and the healthcare team
5. Maintaining detailed records and documentation to support the continuity of care
6. Collaborating with the multidisciplinary team to identify and address any barriers to patient wellbeing
7. Providing emotional support and guidance to patients and their families throughout their healthcare journey

What we're looking for

1. Minimum 1 year of experience as a Case Manager or in a similar healthcare coordination role
2. Strong understanding of healthcare systems, policies, and regulations
3. Excellent communication and interpersonal skills, with the ability to effectively liaise with various stakeholders
4. Proven track record of advocating for patient rights and ensuring high-quality care
5. Exceptional organisational and problem-solving skills, with the ability to multitask and prioritise effectively
6. Empathetic and compassionate approach, with a genuine desire to support and empower patients
7. Relevant healthcare-related qualifications, such as a degree in Nursing, Social Work, or a related field
