Sarcomas are rare tumours accounting for 10% of all paediatric tumours, but still very aggressive for children and adolescents. Extracellular Vesicles (EVs) isolated from Mesenchymal Stromal Cell (MSC) secretome represents a new model for drug delivery, as it allows a controlled and selective release towards the tumour, increasing the biological effect, but limiting toxicity.
Recent studies at the Stem Cell Transplantation Laboratory have demonstrated that the EVs isolated from MSC secretome, loaded with chemotherapy drugs, have a cytotoxic action on osteosarcoma lines, and a new drug delivery system (DDS) approach could be used. For clinical application, it is necessary to characterize the EVs loaded with chemotherapeutic drugs, conduct in vivo studies on murine and canine osteosarcoma models, and propose a phase I study for the treatment of paediatric osteosarcoma patients.
