Job description

We are excited to be advertising for a practitioner to continue St. Margaret's success in wound care & education.

Here at St. Margaret's, we pride ourselves on the holistic approach to Tissue viability and the ultimate goal of comfort and dignity. Patients are often living with complex wounds, malignant or fungating that have a devastating impact on the quality of lives. You will play a vital role in supporting them.

This is a permanent, part time role, working 30 hours per week. In return, you will receive a salary of £38,644.83 to £46,535.67 pro rata, depending on experience.

This role has a working pattern of 7.5-hour shifts, covering the service, 4 days per week. You will be based at the Taunton Hospice, but your reach will be across Somerset and the Inpatient unit. The ability to drive with your own transport is essential to the post but you will also be communicating with patients by video and telephone.

Main duties of the job

You will be responsible for the comprehensive wound care of palliative patients & those with life limiting illnesses under the care of St. Margaret's Hospice.

Your day-to-day work will include:

  • Assessing the holistic needs of our patients, enabling them to live well with the complexity of wound type.
  • Working to develop plans of care and have access to a wide range of specialists within the hospice and external partners.
  • Auditing outcome measures and producing quarterly reports.
  • Providing professional input into the wound care formulary and policy.

Job responsibilities
  • Applying expert knowledge to the comprehensive assessment of patient needs promoting the ability to live well with compromised skin integrity whilst minimizing risk.
  • To practice & promote a high level of quality in infection prevention standards.
  • Demonstrate care, compassion, competence, communication, commitment & courage whilst working to evidence based practice, policy & formulary.
  • Participate in research studies in wound management across and on behalf of the Hospice.
  • Produce accurate audit reports & manage action plans pertaining to improvement & development.
  • Provide complex wound care advice by telephone, video consultation or in person, working to ensure multidisciplinary working relations are maintained. This may require liaising & working directly with District Nursing services or other external services.
  • Engage in multiagency working to ensure that the patients are central to care provision & that boundaries in service provision are clear. Acknowledge the Somerset Tissue Viability Service are commissioned to provide expert advice across Somerset.
  • Deliver both internal & external teaching as required working with the Educational team to address opportunities and need.
  • Support the knowledge development of students attending St. Margaret's for placement.
  • Support link nurse development in both the Community & IPU setting.
  • Actively support the safeguarding of adults and children.
  • Participate in regular multidisciplinary reviews.
  • Confident in the use of electronic health records & using other IT frameworks to capture data & present activity.
  • Participate in the Clinical Strategy & KPIs of the hospice.
  • Provide sign posting & assist patients & carers to navigate health & social care services.
  • Be responsible in ensuring we manage the provision of dressings in the absence of a home office license & remain mindful of the budgetary impact (charitable organization).
  • Demonstrate a proactive approach to grant & funding opportunities.
  • An Independent Prescribing qualification is essential to the role but training can be supported in post.
