Job description

A vacancy has arisen within Adult and Health Services for a Team Manager working in the Derwentside and Chester Le Street Social Work Hub as part one of the Integrated Community Mental Health Teams.

WHAT IS INVOLVED?

You will be responsible for the day-to-day management of a Social Work Hub within an Integrated Mental Health Service, providing mental health assessments, treatment and intervention to service users/patients.

Durham County Council and Tees Esk and Wear Valley Foundation Trust are integrated as part of a partnership agreement. The teams comprise of staff from the Adult Care service from Durham County Council and Tees Esk and Wear Valleys NHS Foundation Trust.

The Service was recently remodelled to now include social work hubs with leadership from social care/social work.

You will have day-to-day operational responsibility for the management of the social work hub and social work staff, which provides assessment, care planning, and interventions for service users/patients and carers referred into the Service with social care needs.

You will work closely with the leadership team within the integrated service including health colleagues to provide the best outcomes for service users.

You will have knowledge of commissioning, social inclusion, legal frameworks and an awareness and understanding of the emerging influences in both health and social care provision. You will co-ordinate and oversee the team’s performance requirements on behalf of the Local Authority.

WHAT WILL I NEED?

You should have managerial experience, alongside excellent communication and interpersonal skills.

Required Qualifications: Appropriate professional social care qualification i.e. DipSW or recognised equivalent, plus Social work England Registration.

If successful, you will be required to apply for a Disclosure and Barring Service Enhanced Disclosure.
