We are looking for a Support Worker to join our team at Portway Lifestyle Centre.
This role will involve:
Candidates will need to demonstrate the ability to be person-centred in their approach to supporting individuals who have a variety of social care needs including complex needs and communication difficulties.
Able to demonstrate how to work in a multi-agency approach with other partners.
This role will require an enhanced adults barring DBS check to be completed.
Hours: 37 hours per week - Monday - Friday
To provide compassionate, safe, effective and responsive support, that will assist service users to maximise their independence.
This will involve providing support with a range of personal and practical tasks, that will achieve agreed personal goals, so that wherever possible service users are able to exercise choice and control maintaining their wellbeing.
After following an individual's plans, staff will report back to their manager, giving accurate feedback and recording progress towards agreed outcomes.
To promote and ensure service delivery which values and respects the dignity, diversity, rights and responsibilities of individuals.
To support and participate in people's chosen activities, promoting citizenship and community engagement, including but not limited to activities both in the person's home and community that will promote both physical and emotional wellbeing, volunteering, and life skills development.
