Location: Community Work, based at Queen Margaret Hospital, Fife. Travel around the Dunfermline area.

An Exciting Opportunity Alert! Join our Discharge to Assess team in Dunfermline! Our new discharge to assess service in Dunfermline enables people, following a stay in hospital, to be supported and assessed in their own home to determine the type and frequency of any care and support they may need to live as independently as possible at home.
Are you a dedicated and experienced Support Worker, with a passion for making a positive impact on the lives of individuals? If so, we want you to be a part of our Discharge to Assess team in Dunfermline. Whether you have experience or not, we will equip you with the skills you need to thrive. This exciting opportunity offers a chance to be part of a new service that takes an enablement approach to providing support and conducting assessments in the person’s home.

This is a Regulated Service and you will play a vital role in providing practical and emotional assistance to individuals in their own homes assessing their needs and conducting assessments to determine ongoing care needs.
