Job description

An opportunity has arisen for a part time specialist nurse in the remote monitoring team. This role will be working in the small remote monitoring team.


The remote monitoring team is a new service provided by Dorset Health Care. The service uses a digital platform to monitor patients in their own place of residence, enabling patients to leave hospital earlier or prevent them from being admitted. The aim of the service is to provide remote monitoring to different specialist pathways working with health providers across Dorset.


The remote monitoring team provides a service 7 days a week from 08:30 to 18:00 at present but the service is commissioned to provide an 08:00 to 20:00 service once it is fully established. The applicant will be expected to work weekends on a roster basis. The hours of working can be negotiated at interview to fit the needs of the service.


The post is advertised for someone to work 30 hours per week, but applicants wanting to work fewer hours will also be considered.


The applicant must have good clinical and communication skills in assessing patients. They must be able to reassure patients using remote monitoring and attend integrated MDT meetings as part of the clinical team caring for remote monitored patients.


The post holder will be expected to provide clinical remote monitoring care to patients with a variety of clinical needs and work with other clinical specialists across Dorset to develop the service.


Main duties of the job

Applications from candidates that require current Skilled Worker sponsorship to work in the UK are unfortunately ineligible to apply for this position.


You will need a sound understanding of the current primary care agenda, including the management of long-term conditions, along with excellent communication skills and the ability to demonstrate effective leadership, decision-making skills, and resilience. You will provide high-quality clinical care, management of staff, and case management for patients in the community setting.


You must be able to demonstrate a high level of clinical knowledge, team working, good interpersonal skills, and time management. You should have experience in caseload management and be able to prioritize work. The role is diverse, challenging, and extremely rewarding.


Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.

Managerial Supervisor Experience
Essential
  • Experience at Practitioner Band 5 level
