Qualifications

Job description

This is an opportunity to work in a dynamic and exciting intermediate care service based in Ealing, West London. As a specialty doctor you will be providing care on one of the 3 bedded rehabilitation wards at Clayponds Hospital or the community based Rapid Response Team (providing Urgent Community Response services).

The role involves working with a consultant led multidisciplinary team to manage patients who have been discharged from or at risk of admission to an acute care setting. You will be delivering high quality patient centred care to manage the daily medical needs of inpatients and those on the caseload.

As a specialty doctor you will be, in addition to your clinical duties you will be actively encouraged and supported to participate in delivering teaching undertaking audit, and in service development.

The role does not require any night time working, but will require a commitment to a 1 in 6 weekend rota (weekend working to be established in the future).

Main duties of the job

The role will include working with a consultant led multidisciplinary team to provide clinical care in bedded units as well as patients in their own residence. The team consists of consultant geriatricians, advanced nurse practitioners, physiotherapists, occupational therapists and care workers in addition to auxiliary staff.

On a rotating basis, the post holder will be expected to participate in the daily virtual ward rounds, home visits, daily ward rounds of bedded wards, liaising with specialist teams, primary care clinicians and other agencies.

The post holder will be required to provide support to nursing, pharmacy, therapy, social work and non-qualified staff to provide clinical care to patients.

Areas of Intermediate Care:

Ealing Rapid Response service - a consultant led multidisciplinary team which aims to manage patients in crisis at home and facilitate admission avoidance.

Clayponds Hospital - a 70 bedded rehabilitation unit across three inpatient wards providing general rehab, stroke/neuro rehab (level 3) and admission avoidance beds.

The post holder will engage with Audit and Service Improvement activities.

The post holder will need to demonstrate working experience in the NHS in the previous 3 years and would ideally suit those with experience of Geriatric Medicine/GP/General Medicine.

  • Shall have completed at least 4 years full-time postgraduate training (or its equivalent gained on a part-time or flexible basis) at least 2 of which will be in a specialty training programme in a relevant specialty or as a fixed term specialty trainee in a relevant specialty or shall have equivalent experience and competences
