The salary for this permanent Social Worker job is up to £48,060 per annum.
The team is the first point of contact for service users, carers and organisations wishing to refer into Adult Social Care for advice, information and community assessment and review.
Working alongside Occupational Therapists and Social Care assessors you will be utilising a strength-based approach and the three conversation model to promote independence and well-being. You will work in line with the Care Act to promote service users choice and control in their daily lives while respecting the key elements of the act to ensure service users can contribute to society and continue to have meaningful, autonomous and self-determined lives.
Due to the complex nature of the role you must be able to communicate clearly and effectively both orally and in writing, producing timely assessments and appropriate support plans. You will have the ability to gather and record information in order to prioritise queries, resolve problems and signpost to alternative services. You will be highly motivated and familiar with dealing with complex and emotionally challenging situations through to resolution while ensuring service users and carers are supported with dignity and respect and are promoted to fully engage in the assessment process, in line with the principles of personalisation and self-directed support.
