Key Responsibilities:

1. Conduct comprehensive assessments of adults with care and support needs to determine eligibility for services.
2. Develop, implement, and review person-centred care and support plans that reflect the needs, preferences, and goals of the individual.
3. Provide advice, support, and guidance to individuals, their families, and carers, promoting independence and choice.
4. Safeguard vulnerable adults and ensure that risk assessments and appropriate interventions are put in place.
5. Work with multi-disciplinary teams, including healthcare professionals, to deliver coordinated care and support.
6. Monitor the effectiveness of care packages and services, making adjustments where necessary to ensure optimal outcomes.
7. Maintain accurate records and documentation in line with statutory and regulatory requirements.
8. Contribute to the development of adult social care services and improvements in practice.
