Job description

Social Care Assistants (Community based)

Location: Buckinghamshire, County Wide

We are looking for dedicated individuals who can provide high-quality direct care and support to residents, focusing on personal wellbeing outcomes and individual wishes to achieve strength-based support.

Positions Available:

  • Part-time and full-time roles available. Please specify your preference in your application.

Work Patterns:

  • Full-time: 37 hours per week, including alternate weekends and shifts between 7 am – 10 pm.
  • Part-time: To be agreed with the hiring manager, but will include every other weekend.

Additional Benefits:

  • In addition to the advertised salary, you will receive a ‘Golden Hello’ one-off lump-sum payment of £1000 upon successful completion of your probationary period. This payment will be pro rata'd for part-time hours.
About us

The Home Independence service supports individuals to regain their independence following illness, disability, or recent stay in hospital. We work with clients in their homes, identifying difficulties with self-care or performing daily activities. We provide a goal-based assessment for each client and support the achievement of their goals, using technology-enabled care devices and occupational therapy aids.

The team works closely with healthcare professionals, carers, clients, and their families to develop suitable packages of support designed to enhance client independence and meet their needs. We supply advice on specialist equipment options, monitor client progress, and make necessary adjustments if required.

About the role

No two days will be the same as you meet a variety of people and support their individual needs. Responsibilities include:

  • Providing information and advice in relation to care and support and assistance in accessing care and support through various means.
  • Working as part of a CQC Registered service in the community, maintaining a high degree of autonomy and utilizing initiative in delivering compliant care and support independently.
  • Ensuring an agreed plan meets individual care and support needs and empowering individuals to access early intervention and preventative services.
  • Informing senior colleagues and adhering to safeguarding procedures for any concerns regarding the safety of vulnerable adults.
  • Working in partnership with individuals, carers, service providers, the voluntary sector, and other professionals.
  • Providing personal care for individuals, which may include dressing, toileting, washing, and general housekeeping.
About you

You will be a great people person, caring and empathetic, able to deliver practical care and support to an agreed plan. You will use your initiative to problem-solve and utilize remote management oversight where required.

You may need to undertake a specialist role that requires specific training, such as Autism services or administering medication. Full training will be provided, and there are opportunities to progress within the service.

Other information

Please note, these positions are community-based, not day centre-based. A full UK driving licence and access to a vehicle are essential. The post holder will be entitled to a car user allowance and a 45p per mile mileage claim allowance (HMRC mileage allowance).

You will be required to apply for a DBS at an enhanced level. Further information about the DBS scheme can be found at GOV.UK.
