An established industry player is seeking an experienced Social Work Practitioner to join their Hospital Discharge Team. We are looking for an experienced Social Work Practitioner who will be our Social Work Senior Practitioner in the hospital discharge team.
As a senior practitioner you will work directly to the Team Manager for Mental Health and the Hospital Discharge Team within Blaenau Gwent.
Access to a vehicle for work purposes is a requirement and the successful post holder will be designated an agile worker which will mean that they will work both at our Social Care buildings and from home.
