Senior Primary Care Liaison Nurse

The CHCP Wellbeing Primary Care Liaison Service are offering a fantastic opportunity for a candidate to join their team on a 12 month temporary/secondment basis as a Senior Primary Care Liaison Nurse to cover maternity leave. We are looking for exceptionally motivated, dynamic and enthusiastic individuals with appropriate experience to join our team within City Health Care Partnership Community Interest Company (CHCP CIC) Wellbeing Primary Care Liaison Service. The successful applicant must be flexible, a good team worker and used to working under pressure. They will be required to have excellent interpersonal skills and be able to effectively communicate with patients.

Main duties of the job

Main duties of the role include:

• Providing nursing and clinical leadership to the wellbeing team, including deputising for the Lead Nurse when required and undertaking the line management and clinical supervision of staff as required.
• Acting as a source of specialist information/advice regarding annual health checks for people with learning disabilities and serious mental illness, support for accessing primary care services and implementing reasonable adjustments where patients are facing barriers to accessing health care.
• Completing clinical audit work on GP registers such as the learning disability register and serious mental illness register, making sure they are cleansed, accurate and any patients missing from registers are identified and added.

Person Specification

Qualifications

• Professional Nursing Health Qualification, RNLD, RGN, RMN, RN
• Dual trained in learning disability and mental health

Knowledge

• Significant post qualification working with patients who have learning disabilities and serious mental illness
• Use of IT equipment, to produce reports and audit
• Previous experience of working with patients with mental health and learning disabilities
• Professional accountability
• Knowledge of annual health checks & health inequalities relating to serious mental illness and learning disability
• Knowledge of psychotropic medication
• Previous experience of working in Primary Care and/or secondary care
• Knowledge of research and audit methods

Experience

• Time management
• Experience of working with mental health users
• Caseload management
• Ability to carry out accurate holistic assessments
• Understand and identify the complex determinants of health inequalities for patients with Serious Mental illness and/or Learning Disabilities
• Team working- able to contribute positively to immediate team and develop links with primary care teams referring to the service
• Can operate as an autonomous practitioner
• Experience of working with groups, and producing and delivering training sessions to varied audiences
• Experience of mental health and learning disability issues leading to the use of mental health services at a Primary Care level
