Job Summary

The Older Persons Assessment & Liaison (OPAL) team are looking for a motivated and dynamic Physiotherapist to join the team on a fixed term secondment. We are looking for someone with an ethos of continual improvement and development to help us provide appropriate and tailored person-centred care to our patients.

This is a fantastic opportunity to join the award-winning OPAL team at Salisbury NHS Foundation Trust. The OPAL team consists of Advanced Clinical practitioners, Occupational Therapists, Physiotherapists and Therapy Assistants. The team work closely with the Frailty MDT to provide specialist assessment and treatment to patients with Frailty within the emergency department, and acute medical unit within a same day emergency care (SDEC) model.

The service prides itself on its success rate and is committed to providing opportunities to develop both clinical and non-clinical skills through our Mentor and Coaching programmes.

Main Duties of the Job

  1. To provide specialist assessment and treatment to patients with Frailty within urgent and emergency care services. Must be able to work in a generalist manner to ensure continuity of care.
  2. To work closely with other members of the Frailty MDT in providing acute hospital-based assessment for frail older people, including Comprehensive Geriatric Assessment.
  3. As an experienced Physiotherapist, to provide therapy advice and support to other disciplines or services, within the team and across the Trust.
  4. To work collaboratively with the wider Frailty Therapy Team to ensure operational and clinical priorities are delivered, to ensure safe patient care.
  5. To extend the scope of clinical practice to enable the support of medical and nursing colleagues to provide excellent patient care.

Person Specification Requirements

Essential

  • Experience of Frailty/front door services
  • Experience of supervising junior staff or students
  • HCPC registered Physiotherapist
  • 1+ years' Experience of working with acute, frail patients

Desirable

  • 'Front door' working experience with a focus on discharge
  • Experience of Service Improvement Projects
  • Cross professional working with OT - experience with Equipment, capacity assessments and functional assessments
