The Spalding Community Mental Health Team is committed to empowering those experiencing severe and complex mental health problems to regain personal autonomy and a sense of active control over their experiences that enables them to return to a full and active life in accordance fulfilling their personal aspirations.

In order to support individuals in achieving this, the team provides comprehensive and multidisciplinary bio-psychosocial assessment and case management. It forms part of the Spalding & South Lincolnshire Locality mental health provision and is actively engaged in the community mental health transformation agenda. We are also actively working to develop the range of effective and evidence-based social and psychologically informed trauma-informed interventions. The successful candidates' values and enthusiasm will be in keeping with this transformation.

The successful applicant will be responsible for the planning and implementation of evidence-based interventions in collaboration with a defined group of service users and their social networks.

Main duties of the job

1. To provide a high standard of collaborative, trauma-informed, strengths-based intervention, focusing on the goals and aspirations of the individual and their social network.
2. To identify clear recovery focused goals utilizing Iroc, the HOPE toolkit, and other appropriate measures to guide the team's interventions.
3. To empower individuals and their social networks, utilizing their strengths in order to achieve their recovery.
4. To work collaboratively with partner agencies across the wider health and social care locality provision in order to meet the needs of social networks.
5. To act as a designated Lead Healthcare Professional ensuring up-to-date collaborative care plans, risk assessments, and reviews are in place.
6. To provide high-quality learning and development opportunities for both colleagues and students.
7. To contribute to the maintenance of a psychologically safe environment in which the diverse views and experiences of all are valued and utilized.

Job responsibilities

1. To engage in comprehensive biopsychosocial assessment of the needs, strengths, and goals of the individual and their social networks.
2. To identify clear recovery focused goals utilizing Iroc, the HOPE toolkit, and other appropriate measures to guide the team's interventions.
3. To facilitate collaborative interventions with individuals and their social networks in order to promote their empowerment, recovery, and wellbeing.
4. Comply with and carry out safe practice in accordance with Trust policies and procedures, appropriate Codes of Conduct, the Mental Health Act (1983), and other relevant national and local guidance commenting on policies, procedures, and developments as necessary.
5. To deliver interventions to promote the individual's physical health and wellbeing.
6. To provide both formal and informal education and health promotion to the individual and their social network regarding their mental health needs.
7. To actively participate in the delivery of training opportunities for staff and students, including those from other teams and disciplines.
8. To provide high-quality practice placement education for students acting as a mentor as required.
9. To contribute to the maintenance of a psychologically safe environment in which the voice, skills, and experience of all are valued and nurtured.

Person Specification

Qualifications

• RMN or RNLD or RN with 1st Level Registration NMC or degree or equivalent. Or Registered Occupational Therapist or Registered Social Worker with HCPC
• ENB998 / FliPS or equivalent
• Evidence of specialised continued professional training (degree level) in clinical practice
• Management training
• CBT, DBT or other relevant psychological therapy training

Experience

• Substantial experience of working with adults with mental health needs and evidence of working with individuals in crisis
• Experience of working with people with severe and enduring mental illnesses
• Evidence of continuing professional development
• Experience working within community mental health settings

Skills

• Sound knowledge of the national agenda for mental health.
• Highly developed clinical reasoning skills.
• Sound knowledge of clinical/risk assessment and understanding of Information Governance principles.
• Highly developed communication skills.
• Delegation whilst maintaining overall responsibility for patients' care, where appropriate.
• Demonstrate the ability to lead a clinical team.
• Highly motivated & able to engage with people accessing the service & their carers to improve outcomes.
• Ability to work independently and collectively.

Special Requirements

• Able to travel around the county independently and in a timely manner.
