Job Introduction

As a Senior Health Care Support Worker you will play a key role in delivering high-quality, person-centered care and services to support individuals and teams. Working within a collaborative environment, you will provide essential services tailored to meet the unique needs of those we serve.

Based in Swindon, you will have the flexibility to work within a model that supports both on-site and hybrid working arrangements, depending on the nature of the role. Main Responsibility

1. To implement treatment by following care plan.
2. To keep the patient at the centre of his or her care, ensuring patient and carer participation.
3. Working within a multi-professional team to provide delegated care.
4. Delivering treatment and support as outlined in a care plan under the guidance of a registered practitioner.
5. Keeping patients at the center of their care, ensuring active participation from both the patient and carers in decision-making.
6. Supporting daily care needs for assigned patients in their home environments.
7. Providing treatment and monitoring of patients as per care plans, following established guidelines and protocols.
8. Reducing unnecessary hospital admissions and supporting early discharge by providing timely and appropriate care.
9. Maintaining clear, concise, and accurate documentation within an electronic patient record system.
10. Recognizing deteriorating patient health and escalating concerns to the appropriate registered practitioner.

The Ideal Candidate

1. Level 3 (SCQF 6) City and Guilds, NVQ 3, BTEC National Diploma or equivalent experience.
2. Level 2 functional skills in English and Maths.
3. Completion of Care Certificate.
4. Experience of supporting other workers.
5. Experience working independently under the delegation of a registered nurse or paramedic.
6. Ability to prioritize work and demonstrate effective time management.
7. Committed to a caring approach with patients and relatives.
8. Strong communication skills, both written and verbal.
9. Commitment to providing high-quality care.
10. Full driving license and use of a car during working hours.
