Job Description:

We have been assigned by one of the UK's more prestigious NHS Trusts to help them recruit a Senior Clinical Fellow in Acute Medicine on either a Substantive or an NHS Fixed Term basis for 12 months to join their successful Medicine department.

Our client's ambulatory areas are designated areas located within their two emergency villages which are used to identify, investigate, and manage emergency admissions arriving in AE or from primary care providers who can be managed and discharged within an 8-hour period. These patients ideally are those who will not need inpatient admissions, thus facilitating admission avoidance and relieving the strain experienced by our accident and emergency department. A number of designated pathways and processes underpin the clinical decision-making done by our senior clinical decision makers in the ambulatory area. At both sites, our client aims for direct consultant input in the ambulatory area to facilitate the effectiveness of these areas and we hope that all new team members will embrace this.

Their medical assessment units are designed in a zonal setup which will allow developments such as frailty or high-intensity care areas with individuals requiring monitoring. The aim is to facilitate prompt clinical decision-making with real-time post-taking to ensure a clear plan is formulated to establish the mantra of the ‘right patient, right bed, under right specialty if required' and that within 72 hours from EDD (estimated date of discharge), the patient reaches the right destination, be it home, ward bed, or other discharge destinations. Their acute medicine unit is an area for education and learning with medical students, advanced care practitioners, nursing staff, physician associates, and other allied health care advisors working, learning, and collaborating daily.

The post-holder will provide registrar-level experience in a teaching hospital with all the main medical sub-specialties. This role would particularly suit doctors who have completed CMT and individuals with equivalent experience who are looking to progress to specialty training, as well as those looking to pursue a non-consultant career grade or SAS route. The clinical fellow will be an integral part of the junior medical workforce involved in the clerking and management of all patients in our acute medicine and ambulatory environment. The post is for 12 months and will provide excellent training in the management of common and important presentations normally seen in acute medical settings and give insight into the management of acutely ill patients. The post would be excellent preparation for a specialist training position.

The successful candidate will undertake the following roles:

  1. Clerking of acute medical patients in both ambulatory and medical assessment units
  2. Clinical reviews of patients on both the ambulatory unit and medical assessment unit
  3. Participating in daily ward rounds on the medical assessment unit
  4. Assisting on real-time post-take ward rounds with senior decision makers in accident and emergency, medical assessment unit, and ambulatory unit
  5. Participating in continuous improvement projects, education, and audit
