Job description

An exciting opportunity has arisen with a senior care co-ordinator position in the region wide Complex Care Social Work Team. This post offers the opportunity to contribute to service delivery and development within Adult Services on a single and multi-agency/disciplinary basis. The appointed candidate will be based either in the Stranraer or Castle Douglas office and the team supports hybrid working.

You will work 36 hours per week over 52.14 weeks per year.

Your primary responsibility will be to assess the needs and risks of/to service users in the community or hospital. This is done in conjunction with service users, their families &/or carers and partner agencies including health and police. Knowledge of community services for adults in the statutory, third and independent sectors would be beneficial.

Experience of working in a health or social services setting is essential and a good working knowledge of the Mosaic system is desirable. Experience of assessment and care management is desirable, as well as an SVQ Level 4 or equivalent in Health and Social Care.

You will be expected to practice and demonstrate good knowledge of the application of the Self Directed Support (Scotland) Act 2013. A good general knowledge of the parameters of the Adults with Incapacity (Scotland) Act 2000 and the Adult Support and Protection (Scotland) Act 2007 is also desirable.

You will need to demonstrate excellent communication, assessment, IT skills, case management, analytical and co-ordination skills and be able to work under your own initiative and prioritise accordingly. You should have a good ability to complete assessments of need and risk and be able to prepare plans that flow from these within time frameworks. You will be committed to evidence-based practice and will receive good support, regular supervision and opportunities for development.

A current driving licence is an essential requirement of this post and you will be expected to use Dumfries and Galloway Pool cars for any travelling required. However, if you have a disability which precludes you from holding a driving licence consideration will be given if alternative transport can be arranged.
