St Margaret's Hospice Care has a fantastic opportunity for a Senior Access Team Practitioner to join our Access team.
Based in our Central Referral Centre, you will manage phone inquiries from patients and health care professionals to our Advice Line, triaging referrals and responding as appropriate. You will play a vital role in the transformation and development of an Access team that is clinician led.
This role is based at our Hospice site in Taunton. This is a full time, permanent role working 37.5 hours per week. In return, you will receive a competitive salary of £37,338 - £44,962 per annum, depending on experience.
This role offers a working pattern of 7.5-hour shifts, over 7 days per week on a rota system. As this is a role supporting transformation unsocial hours may be introduced later so to begin with supporting the working week will be a priority.
Part of the leadership team, you will help drive forward a revised model of working, focusing on our referral, triage and advice line.
You will work closely with the Head of community services, our inpatient unit, existing Central Referral Centre and the wider hospice and external services.

Support all aspects of the clinical referral process following referral criteria, triaging each referral and identifying response and needs based on assessment skills.
Assess the appropriateness and urgency of referrals seeking further information and advice as appropriate managing expectation and giving clarity to outcome.
Assessing, planning and implementing care within a holistic model, both remotely and face to face, effectively identifying the needs of patients and those close to them.
Supporting and advising patients, using specialist knowledge and skills in physical symptom control, social, emotional and spiritual care.
Working with methods of quality improvement and encouraging others to do so to ensure change is safe, well led and responsive.
Working collaboratively with the hospice multidisciplinary and external teams to provide care, support and direction.
Providing advice, support and education to other health care professionals, where appropriate, as part of a multi professional approach.
Liaising with other teams within St Margarets hospice care following triage and assessment process.
Manage advice line calls and act as the first line of support and advice.
