What you will do:

• Work autonomously in patients’ homes, conducting assessments and providing tailored rehabilitation plans for individuals who have been referred for physiotherapy intervention.
• Deliver high-quality care and continually assess the effectiveness of interventions.
• Work collaboratively with a diverse team of healthcare professionals to ensure comprehensive patient care and service delivery.
• Document patient progress meticulously and report any changes in condition to the appropriate healthcare team members.

To excel in this role, you must be HCPC registered and possess a full driving licence, as this role requires travel between patients’ homes.
