As part of an expanding workforce working with rough sleepers, an exciting opportunity has arisen to support people with their mental health who are rough sleeping, in temporary accommodation, or at risk of losing their accommodation.
You will work within a developing team of nurses who work alongside external agencies to ensure rough sleepers have the care and support they require. You will actively look to continue building relationships with agencies and be proactive in delivering therapeutic intervention to service users. You will ensure that a high quality of care is delivered within this clinical area. You will work autonomously, think creatively, and be able to manage the UK weather as we hold a variety of visits in a variety of places!
The rough sleeping team continues to grow and develop; this is an exciting opportunity to be a part of a team where you can have input on the development, expansion, and service delivery. Working and supporting some of the most vulnerable ensures that everyone in society has access to the services they require. The role itself is challenging and changeable but ultimately very rewarding.
The team delivers services to the population of Shropshire, Telford, and Wrekin. The base for the post is flexible and can be negotiated with the service manager. A level of flexible and remote working is supported.

You will provide clinical knowledge and interventions, advice, and support to rough sleepers across Shropshire. You will undertake rapid assessment and support to adults 18+ who are homeless or rough sleeping. You may also be called upon to provide mental health support to adults who are housed and supported by the local authority but whose tenancy is in jeopardy due to a decline in their mental health.
You will be expected to participate in the multi-agency RESET meetings to provide guidance, signposting, and clinical knowledge to the MDT. The post holder will be responsible for providing evidence-based care and interventions and providing professional nursing leadership within the pathway team.
The RESET team is made up of the Shropshire Council, With You at Shropshire Recovery Partnership, Midlands Partnership Foundation Trust (MPFT), The Shrewsbury Ark, and Intuitive Thinking Skills.
The RESET team provides the following services to Shropshire's rough sleeping community: Drug and alcohol recovery support and treatment including medical prescribing via With You at Shropshire Recovery Partnership. Learning new skills or new coping strategies via Intuitive Thinking Skills training, to improve people's resilience and support them to address their substance use with confidence. Mental health treatment and support provided by the MPFT. Ongoing support and day centre facilities provided by The Shrewsbury Ark.
