We are searching for a talented RN Supportive Care - VNA
*$12,000 new hire sign-on bonus for this position. (rehires termed more than 1 year also eligible)*
Hours: 40hrs
Shift:
Location: Visiting Nurse Association - Fairhaven, MA
**Available to regular status employees who are scheduled to work a minimum of 20 hours.

Under the supervision of the Director, Supportive Care Services or designee, and within agency and department policies and procedures, assume responsibility for the primary care of assigned Hospice and Palliative Care patients and families. Assess, develop, implement, and evaluate an individualized plan of care to maximize the comfort and health of the Hospice and Palliative Care patient and family. Responsible for identifying patient/family needs and for providing supportive care in accordance with the attending Physicians orders, Plan of Care, and Hospice and Home Care policies and procedures.
Compensation: Pay rate will be determined based on level of experience.
USD $37.82 - USD $53.96 /Hr.
