Young People and Families

Our Young People and Families hub works with those in trouble, providing early intervention, and targeted and specialist support services to those who are in crisis, leaving care, missing from home, or with substance misuse or mental health problems. Services support people no matter what their situation – and have demonstrated a high success rate.

Our Merton Risk and Resilience Service is an integrated service focused on mitigating risk and promoting resilience in young people. The service aims to reduce harm to young people aged 24 and under, who are at serious risk of harm from, or at risk of, harm from using substances, going missing from home or care, and being exploited.

Job Description

The role of Risk and Resilience Practitioner is an excellent opportunity to provide tailored support and guidance to children, young people, and families. You will undertake one to one work with service users, assessing their needs, and determining appropriate support plans and pathways, to inform risk reduction plans.

Working in close partnership with a range of different partner agencies and commissioners, you will also have the opportunity to build networks and knowledge within the Children’s Social Care Sector.

We will also support your development, to build a specialism in services such as Substance Misuse, Child Exploitation, Missing from Home, or Families work.

*Access to a car, and a full driving licence, is a requirement of this role.

Main Duties and Accountabilities

  • Determining a range of appropriate support mechanisms, services, plans, and pathways for a caseload of young people, and informing a risk reduction plan, to reduce risk factors, enabling all service users to fulfil their potential.
  • Ensuring that young people are provided with appropriate needs-based support - including interventions, direct one to one work, vulnerability assessments, and return home interviews, and supporting access to additional or specialist services where necessary.
  • Developing and sustaining productive working relationships with referring agencies, local authorities, children’s social care, and Children and Families services (CFS), with effective liaison and communication to ensure high quality referrals, support, care and risk management, ease of transition, and promotion of continued joint service work.
  • Planning and delivering support interventions in accordance with commission, local authority, and Catch22 KPIs, policies, and procedures, ensuring the effective use of resources and the achievement of targets.
  • Ensuring that all data such as service user information, monitoring information, and interventions, are recorded appropriately by set timescales, alongside tracking and monitoring outcome progress and KPI targets, and assisting the production of feedback reports.
  • Ensuring meaningful service user participation and active co-production, and providing consultation, information, training, and presentations, to professionals, family members, and young people, from a range of settings.
  • Contributing to achieving high standards of care, quality, and positive outcomes, managing your time and activities safely and effectively, to meet priorities and delivery requirements.
Qualifications
  • Minimum 5 GCSE's, grade C or above, or equivalent.
  • Experience of working with children and young people with complex needs or vulnerabilities.
  • Knowledge of substance misuse treatment, child exploitation, or missing from home guidance and methodology, and relevant policies and practice.
  • Access to a car, and a full driving licence.
Additional Information

Salary: Up to £29,000 per annum (depending on knowledge and experience)
Hours of work: Full time, 37 hours per week (occasional evening and weekend working may be required)
Contract: Permanent

Location: This post will be based in our Morden hub, but will involve travel around the London Borough of Merton on a daily basis.
Flexibility: Occasional home/remote working is available, but this is dependent on service requirements, so we are unable to guarantee specific days.
